Plan Evaluation

A comprehensive plan evaluation is delivered over 3 to 6 weeks, and involves up to 3 one-hour consultation sessions.

This service is suited to:

  • participants with an existing NDIS plan needing support to implement their plan effectively

  • participants with complex needs, service barriers, or funding shortfalls

  • participants needing assistance with reparation for plan reviews or appeals

  • individuals seeking NDIS access who have received a rejection

This evaluation is designed to improve your understanding of your plan and how to use it efficiently. We review your plan documents, analyse budgets, assess supports, and develop a tailored action plan to help you implement your plan with confidence.

the evaluation process:

Schedule your first one-hour consultation using our online booking platform and upload your initial documents* to be reviewed by our team. You will need

  1. NDIS Plan OR Access Document

  2. Up to two optional supporting documents, such as diagnosis letters, allied health reports, behaviour support plans, medical or carer impact statements.

*Additional documents may be requested. The minimum requirement for this service is an exisiting NDIS Plan, or your completed Access Document

Based on the first consultation and documents you provide, our team will prepare a report over the next 3–6 weeks, depending on complexity. You will receive a summary report outlining clear suggestions, support options, and action steps.

After a follow‑up consultation to clarify and refine next steps, the final report will be provided. The completed evaluation is tailored to your unique situation, including references guide and a step-by-step action plan to support your future NDIS journey.
